Homemaker:
This service provides qualified workers who help with routine household tasks to maintain a clean and healthy environment for older individuals with functional limitations.

Information and Assistance:
Assistance to individuals in finding appropriate service providers that can meet their needs including information-giving (e.g., listing the providers of services so an individual can make their own direct contact) and referral (making contact with a provider on behalf of an individual and follow-up contacts to clients).

Home Delivered Meals:
Referrals are made to Northwest Michigan Human Services for those homebound seniors who are unable to prepare food for themselves.

Medical Transportation:
Medical transportation for those seniors who are unable to get to medical appointments. This service is provided at no cost but donations are accepted. We use BATA and need one week notice.

Home Chore:
This is a service of the LCCOA in cooperation with area Home Chore providers (must have an Agreement signed with Leelanau County). It is designed to give senior citizens assistance in paying for Home Chore services (Lawn Maintenance, Snow Removal & Window Cleaning).

Senior Companion Program:
This program is a national adult volunteer program in which seniors help other seniors maintain independent living. Volunteers visit assigned special friends on a weekly schedule providing one-to-one companionship and practical assistance.

Unmet Needs:
Income must be at 100% of poverty level or lower. Requests are honored for emergency situations and only after all possibilities for assistance have been exhausted. Requests are honored one time only per year per client. Requests are honored for $300.00 or less. Any amount over $300.00 is the responsibility of the client.

Personal Care:
Provision of in-home assistance with activities of daily living (ADL) for an individual including assistance with bathing, dressing, grooming, transferring, toileting, and ambulation. Personal care does not include any other health oriented services as specified for Home Health Aide Services.

Respite Care:
Provisions of companionship, supervision and/or assistance with activities of daily living for clients that must require continual supervision in order to live in their own homes or the home of a primary caregiver. Clients may have difficulty performing or be unable to perform activities of daily living (ADL) without assistance.
