F.         IN ADDITION TO THE ABOVE INFORMATION, THE FOLLOWING ITEMS MUST BE SUBMITTED WITH EACH STAGE 1 - PSO APPLICATION:

 

1.                     A map of the proposed PSO site showing:

a.                                 site size, indicating property boundaries with dimensions           

b.                                 adjacent roads

c.                                 natural features, such as woodlands, dunes, streams, ponds, wetlands, orchards, etc.

d.                                 topography

e.                                 non-buildable areas, such as wetlands, dunes

f.                                  existing zoning.

 

2.                     A sketch plan showing a general layout of how the applicant intends to use the property (lot locations, roads, open space, etc.), and demonstrating at least the minimum eligibility requirements, the intended use of the dedicated open space, and the method of dedication.

 

3.                     A written statement indicating that the applicant intends to meet the minimum eligibility and density requirements, and such other information as will illustrate the applicant's intent.

 

G.            AFTER INITIAL APPROVAL OF THE APPLICATION FOR A STAGE 1 - PSO, OTHER REQUIREMENTS INCLUDE:

 

1.                     An Application for Land Division Act approved by the Township

2.                     If applicable, an approved Community  Sewer Agreement (Township Resolution 9-1 of 1998)

3.                     If applicable, approved Private Access Road plans (Township Ordinance 90-1 of 1990)

4.                     Providing to the Township recorded copies of the Irrevocable Conveyance, Deed Restrictions, Protective Covenants, Conservation Easements, when applicable

5.                     Providing to the Township copies of other regulatory agency permits (as may be required),  including but not limited to:

 

                        Leelanau County Road Commission (County road access)

Michigan Department of Transportation (State road access)

                        Benzie/Leelanau District Health Department (water and waste treatment)

Michigan Department of Environmental Quality (water or waste treatment, wetlands, sand dunes)

                        Leelanau Soil Conservation District (soil erosion control)

Leelanau County Drain Commissioner (drainage control)

                        Any applicable Federal Permits or Authorizations

 

Application is hereby made for a Planned Site Option - Stage 1, pursuant to Article 15 of the Leelanau Township Zoning Ordinance.  I certify that I am familiar with the information contained in the application, and that to the best of my knowledge and belief, such information is true and accurate, and in compliance with Township Ordinances.  I certify that I have the authority to undertake the activities proposed in the application.  By signing this application, I understand to allow a representative of Leelanau Township to enter upon said property in order to inspect the proposed project.
